Testicular contusion

Presentation

Direct testicular trauma on a fight. Painful left testis.

Patient Data

Age: 25 years
Gender: Male
ultrasound

An oval-shaped homogeneous hypoechoic subtunical area is noted at the inferoposterior aspect of the left testis with preserved normal testicular parenchymal vascularity and intact overlying tunica albuginea which is seen thickened. No hydrocele. Preserved oval shape of the left testis.   

Case Discussion

Sonographic features of a hypoechoic homogeneous subtunical area with intact tunica albuginea and preserved normal testicular vascularity and history of acute blunt testicular trauma point to diagnosis of testicular contusion.

Signs of testicular rupture are heterogeneous echogenicity of the testis with abnormal contour of the testis, loss of vascularity and disrupted tunica albuginea. Intratesticular hematoma also shows heterogeneous testicular echotexture and loss of vascularity but with intact tunica albuginea. Segmental testicular infarction would show no internal vascularity.
