Medial protrusion of tibialis anterior muscle noted through a fascial defect of about 18mm in the mid-leg, herniating across the anterior crest of the tibia. Fatty replacement of herniated muscle segment noted.

Case Discussion
Muscle hernias are commonly found in lower extremities. Tibialis anterior muscle hernias are found most frequently due to the vulnerability of its fascia to trauma usually producing a focal bulge.
