Torsion of the appendix epididymis

Normal appearance of scrotum and tunicae.
Testes are of normal size and echotexture (left testis not shown here). Normal arterial flow in both.
Normal appearing epididymis demonstrated bilaterally.
Located between upper pole of right testis and head of epididymis is an elliptical structure with meshlike echotexture, measuring approximately 3.5 x 2.5 mm, devoid of internal flow - most compatible with torsion of appendix epididymis.
No evidence of hydrocele or varicocele.
