Diagnosis probable

Presentation

Cyanosis at birth, respiratory distress.

Patient Data

Age: 20 years
Gender: Male

"Snowman" or "Figure of 8" appearance to the superior mediastinum indicative of supracardiac type of TAPVC.

Dilated central pulmonary arteries are also present with relative pruning peripherally. 

Case Discussion

"Snowman" or "Figure of 8" appearance is sign seen on chest x-rays in the supracardiac type of TAPVC. The patient has developed pulmonary hypertension (Eisenmenger’s physiology) as a result of the right to left shunt existing for many years, this is evidenced by the dilated central pulmonary arteries.
