Presentation

General poor condition, fever and abdominal distension.

Patient Data

Age: 7 years
Gender: Male

Abnormal marked dilatation of the transverse as well as the proximal part of the left side of the colon with loss of haustral pattern. The diameter of the transverse colon measures 84 mm. The ascending colon displays thick walls.

Moderate amount of abdominal free fluid. No abnormal fluid-fluid levels. Normal opacification of the portal vein, no abnormal intraluminal gas. No abnormal gas under the diaphragm.

Abnormal edema of the anterior and posterior abdominal walls more obvious at the level of the groin.

Case Discussion

Toxic megacolon usually presents with fever, tachycardia and leukocytosis. Other manifestations include diarrhea, abdominal pain, rectal bleeding, vomiting, disturbed conscious level and sometimes hypotension.
