Transsphincteric perianal fistula

Left perianal transsphincteric fistulous tract about 4 cm long and arising from 1 o'clock. The fistula can be seen as a T2-hyperintense fluid tract briefly ascending and then curving medially through the external and internal anal sphincters. The tract extends into the ischioanal fossa and decompresses at the skin surface.
Case Discussion
According to St. James University Hospital MR classification system, this is a Grade III left perianal transsphincteric fistulous tract with inflammatory changes of the left ischioanal fossa1.
