Presentation

Thrown from horse against fence. GCS 14/15. Periorbital swelling on the left. Unable to open left eye.

Patient Data

Age: 55 years
Gender: Female

Traumatic dislocation of the lens can be seen with extensive edema and subcutaneous emphysaema of the left periorbital soft tissues.

No intra- or extraconal pathology detected.

There is no obvious vitreous hemorrhage.

Case Discussion

Traumatic lens dislocation is an uncommon complication of head injuries, but blunt trauma in AP direction is the most common cause of lens dislocation due to equatorial expansion and subsequent disruption of zonular fibers.
