The A1 pulley is thickened - there is thickening of the flexor tendons of the right fourth finger at the level of the A1 pulley (right half of right image) compared to the contralateral A1 pulley (left half).
Case Discussion
There was thickening of the A1 pulley. The normal pulley should be less than 0.5 mm in thickness; a thickness of over 1-1.5 mm is considered abnormal. The A2-A5 pulleys were of normal caliber. There was accompanying tendinosis of the flexor digitorum superficialis and profundus tendon complex at A1.
Findings were consistent with the clinical diagnosis of trigger finger and characterized and localized the source of the problem.
