Tuberous sclerosis - incidental finding on trauma scan

Case contributed by Michael P Hartung
Diagnosis almost certain

Presentation

Fall from a tree two weeks prior.

Patient Data

Age: 20 years
Gender: Female

Numerous sclerotic lesions of varying sizes throughout the imaged vertebral bodies, posterior elements, and pelvis.

Comminuted left acetabular fracture.

Bilateral renal angiomyolipomas.

Case Discussion

Tuberous sclerosis is characterized by a variety of hamartomatous lesions in various organs.

This patient has bilateral renal angiomyolipomas, which is commonly associated with tuberous sclerosis. However, a lesser recognized finding is more notable in this examination, which is the presence of numerous sclerotic lesions within the vertebral bodies, posterior elements, and pelvis. This important association can help avoid the mistake of suggested osteoblastic mestastases, particularly in this young patient.
