Undescended testis

Case contributed by Naqibullah Foladi
Diagnosis almost certain

Presentation

For evaluation of left side testis

Patient Data

Age: 17 years
Gender: Male
mri

Left testis is not visualized in the left scrotal cavity, instead, an oval structure is noted returning high signal intensity on T2/STIR sequences at the level of the left internal inguinal ring. The structure is showing intense diffusion restriction in DWI and corresponding ADC maps.
The right testis is normally located in the right scrotal cavity.

Case Discussion

Cryptorchidism can be unilateral or bilateral. The first line investigation is ultrasound. MRI can be a good examination for abdominal testis. The preferred sequences used in the detection of the undescended testis are T1 showing low signal intensity, T2 with high signal intensity, and DWI demonstrating diffusion restriction.
