Unilateral delayed nephrogram - obstructive uropathy

Presentation

Left lower quadrant abdominal pain.

Patient Data

Age: 60 years
Gender: Female

Left vesicoureteric junction calculus with mild left hydronephrosis. 

Contrast from the previous CECT outlines the bladder obscuring the vesicoureteric junctions. Delayed left nephrogram with mild hydronephrosis. Absent right nephrogram. 

Contrast has cleared the urinary system. Left vesicoureteric calculus demonstrated at the level of the ischial tuberosity. 

Case Discussion

This patient presented with non-specific abdominal pain, hence a contrast-enhanced CT was performed. This demonstrated a left VUJ calculus with mild hydronephrosis. An x-ray shortly after the CT to assess for radiopacity of the calculus was performed, demonstrating a delayed left nephrogram and indicates a degree of left ureteric obstruction.
