Unilateral renal agenesis and class II Mullerian duct anomaly

Case contributed by Varun Babu
Diagnosis certain

Presentation

Urinary tract infection, ultrasound did not identify a left kidney. ? ectopic / ? dysplastic.

Patient Data

Age: 25 years
Gender: Female

Two separate horns identified for the uterus with the right horn communicating with the cervical canal. Left horn has an endometrial canal of it's own which doesn't seem to communicate with the right horn or cervix. Appears to be attached to the uterus via a thin band. Between the non communicating left horn and the uterine body is a mutlicystic lesion with varying levels of signal intensity. This lesion could very much represent endometriotic hematocolpos secondary to repetitive shedding of the rudimentary horn. Non visualization of left kidney in the normal position nor in any other ectopic position per abdomen or pelvis.

Case Discussion

Identification of any one urogenital anomaly warrants a detailed search for others. This patient has unilateral left renal agenesis, accompanied by a rudimentary non communicating left horn of uterus.
