Upper limb occlusive venous thrombosis

Case contributed by Dennis Odhiambo Agolah
Diagnosis almost certain

Presentation

Post ligation of left arteriovenous fistula. Now swollen site with pulsations and increased local temperature. To rule out thrombosis.

Patient Data

Age: 60 Years
Gender: Male
ultrasound

Extensive intraluminal venous material (thrombosis) with heterogenous, mixed echo reflectivity and solid predominance involving the proximal left cephalic and basilic veins as well as the left infero-medial subclavian venous system. Notable are hyperechogenic calcific foci interspersed within the thrombus material and there is accompanying regional subcutaneous tissue edema of the left upper arm noted as well. The left forearm vasculature were grossly unremarkable.

Case Discussion

The findings are reflecting left upper extremity occluding, acute venous thrombosis interspersed with subtle calcific foci areas and attendant regional subcutaneous tissue edema ipsilaterally.
