Urethral diverticulum complicated with stone

Case contributed by Ahsan Shahzad
Diagnosis almost certain

Presentation

Patient presented with painful micturation, post urination dribbling and markedly decreased ejaculation volume.

Patient Data

Age: 25 years
Gender: Male

Stricture at the bulbomembranous junction along with a urethral diverticulum communicating with penile urethra is appreciated on retrograde urethrogram. A relatively less radioopaque oval density is appreciated in the middle of sinus likely due to stone on contrast film.

The radioopaque density is also appreciated on plain film in the peri penile uretheric region on oblique and plain films likely due to stone.

Case Discussion

This patient had pre-scrotal and subcoronal hypospadias for which a two-staged urethroplasty was done in the past. His symptoms did not settle and along with decreased urinary stream there was post micturition dribbling from prescrotal hypospadias.  He was evaluated for his symptoms and stricture at the bulbomembranous junction along with a urethral diverticulum was appreciated, which is complicated with a stone that might have formed due to urinary stasis over the period of time.
