Volume loss. Bilateral peripheral and lower zone reticular interstitial densities. Imaging features most match a pulmonary fibrosis with a UIP type pattern

Confirms radiographic findings. Traction bronchiectasis, basal and peripheral honeycombing. Increased attenuation of the lower lobe parenchyma. Underlying paraseptal emphysema noted.
Case Discussion
Further history and examination demonstrated a fever and productive cough superimposed on chronic symptomatology. Parenchymal densities cleared after treatment with antibiotics and the patient improved clinically.
