Peripheral septal thickening, macrocytic honeycombing, and traction bronchiectasis with an apicobasal gradient of distribution. Also noted paraseptal and centrilobular emphysema with large apical bullae suggestive of concomitant COPD. 

Case Discussion

The diagnosis of idiopathic pulmonary fibrosis requires the exclusion of other known causes of usual interstitial pneumonia, including use of toxic drugs, environmental exposure (asbestos), and collagen vascular disease.
