Usual interstitial pneumonia (UIP)

Case contributed by Mohammad Taghi Niknejad
Diagnosis certain

Presentation

Elderly male with chronic dyspnea and impaired PFT (restrictive pattern). Heavy smoker.

Patient Data

Age: 70 years
Gender: Male

Diffuse lung architectural distortion due to patchy areas of intralobular septal thickening are noted resulting in reticular pattern at peripheral and subpleural lung regions asymmetrically in bilateral lung fields with basal predominance.
There is also evidence of dilated and distorted centrilobular bronchioles within the areas of reticulations suggestive of tractional bronchiolectasis. Surrounding areas of fibrosis with segmental and subsegmental bronchiectasis also are observed. Patchy areas of concentric macrocystic honeycombing with peripheral and subpleural distribution are evident bilaterally.
Few enlarged lymphnodes are seen at mediastinum. The largest one measured 22×15mm in diameter.

Case Discussion
