Uterine adenomyosis

Presentation

Pelvic pain, intermenstrual bleeding.

Patient Data

Age: 40 years
Gender: Female

There is bulky uterus, widening junctional zone, ill-defined diffuse hypointensity in posterior myometrium, with multiple hyperintense foci in T2W images, likely small areas of cystic changes and shows heterogenous enhancement in t1_vibe post-contrast. It causes regional mass effect, splaying the endometrial cavity.

Also, there is a tortuous dilated cystic lesion on left side adjacent uterus, appears heterogeneously hyperintense on T2W images, with hyperintensity on T1W images, likely blood content, suggestive left hydrosalpinx.

Case Discussion

Diffusely thickened posterior junctional zone with multiple bright cystic foci in it, suggestive of diffuse uterine adenomyosis.
