Zoster neuralgia facial nerve

Presentation

Acute right facial palsy.

Patient Data

Age: 55
Gender: Female

On post contrast fat-saturated axial and coronal images there is asymmetric enhancement of the right facial nerve, particularly heterogeneous and extending into the internal acoustic meatus. The mastoid segment of the facial nerve is perhaps also slightly brighter than on the left. The remainder of the study is unremarkable in appearance.

Conclusion:

Asymmetric enhancement of the right facial nerve, particularly at the geniculate ganglion and lateral aspect of the meatal segment is consistent with the clinical history of zoster involvement.

Case Discussion

This patient has known and proven Herpes zoster neuralgia with an acute right sided facial nerve palsy.
