The geyser sign is a subcutaneous pseudotumor above the acromioclavicular (AC) joint.

Synovial fluid escapes through a chronic full-thickness tear of the rotator cuff. The joint fluid "erupts" superiorly through a degenerated AC joint into the subdeltoid bursa.
