Which sequences are best for this?
Thin slice T2W, coronal to the temporal lobes, are best to demonstrate structural differences. Increased FLAIR signal can also be seen on the affected side.
Is the left hippocampus normal?
On this scan it is, but beware, up to 10% of patients have bilateral MTS.
Asymmetry of the mesial temporal and hippocampal structures. Loss of bulk and internal architecture of the right hippocampus. Normal temporal neocortex and amygdala.
T2W/FLAIR hyperintensities consistent with chronic small vessel ischaemic disease. Right parafalcine ossification.
