What is the diagnosis?
Biliary obstruction probably due to dead worms (ascariasis) in distal common bile duct.
Why do you think, these are 'dead' and not 'alive'?
While the live worms have a hyperintense tubular gut visible sometimes parallel to long axis of common bile duct ("three line sign" usually seen on 3T MRI), this structure appears hypointense and fragmented.
Curvilinear and irregular serpiginous flow voids are seen in distal common bile duct, with obstructive biliary dilatation..
