Carbon monoxide poisoning

Discussion:

Given this patient had a known history of carbon monoxide poisoning, these radiographic changes, which have characteristic symmetric T1 and T2 signal changes within the globi pallidi, are almost certainly due to the carbon monoxide poisoning.

In a patient without such a known history, other etiologies for bilateral basal ganglia lesions should be considered, including other toxic encephalopathies.
