Look carefully. Does this IVP actually demonstrate the sub-capsular collections?
A double contour of left kidney is evident, due to contrast leakage in subcapsular region.
Kidneys appear hypoenhancing, due to chronic compression by subcapsular collection. This can result in hypertension. What is the mechanism behind this, and what is the name given to such a kidney?
This is known as a Page kidney. Compression of the renal parenchyma results in compression of the renal vessels, which leads to decreased blood flow and activation of the renin-angiotensin system. The renin-angiotensin system activation results in a rise in blood pressure and where this is prolonged, hypertension.
Is this the common age for renal lymphangiectasia?
No. Renal lymphangiectasia is usually seen in children, although adult cases have certainly been described.
Delayed radiograph showing relatively normal excretion on the right and hold up in region of the left PUJ. Note a double contour to the left kidney due to accumulation of contrast in the subcapsular fluid.
