Aside from the kidney, what is the other common origin for a psoas abscess?
Osteomyelitis. Extension of the infection into the paravertebral space is relatively common, especially in TB.
What would be the first line management?
Image guided drainage of the abscess, typically with a CT guided percutaenous drain insertion.
The left kidney is hydronephrotic as well as the proximal ureter with no intraureteric stones indicating ureteric stricture. Associated stranding of the perinephric fat is seen. This is complicated by pyonephrosis, perinephric and psoas muscle abscesses.
