Discussion:

Multilobulated cyst in the tail of pancreas, contiguous with the main pancreatic duct, with a bunch of grapes appearance, consistent with an intraductal papillary mucinous neoplasm of the pancreas (IPMN).

The pancreatic parenchyma is normal - no evidence of chronic pancreatitis.
