What is the most likely diagnosis?
Unicameral (simple) bone cyst.
What radiographic sign is present which can make you quite certain of the diagnosis?
Fallen fragment sign.
Two x-rays of the right humerus demonstrate a lucency within the proximal diaphysis with slight expansion and a fracture. Lying medially within the cyst is a fragment of cortex.
