What is the cause of the patient's presentation?
Dural venous sinus thrombosis with venous infarct in the right parietal lobe.
Which venous structures are affected?
Superior sagittal sinus, straight sinus, vein of Galen, transverse sinus on the left and a number of cortical veins.
How would you confirm the diagnosis and extent of thrombosis?
CT venography is often easiest to interpret and organise in the acute setting. Alternatives are MR venography or catheter angiography (mostly historical).
This non-contrast CT scan of the brain demonstrates hyperdense superior sagittal sinus, straight sinus, vein of Galen, transverse sinus on the left and a number of cortical veins. In addition a region of low density with foci of haemorrhage is noted in the partial lobe on the right.
