Sliding hiatal hernia

Discussion:

Widened oesophagal hiatus with protrusion of gastroesophageal junction and stomach through oesophagal hiatus into right thoracic cavity representing a sliding hiatal hernia.

Low density filling defects of segmental branches of the left lower lobe pulmonary artery and single segmental branch of the right lower lobe pulmonary artery representing bilateral acute PEs

Shallow left sided pleural effusion and middle lobe, lingular and bilateral lower lobe atelectasis.
