What appearance do artery of Percheron infarcts have?
These infarcts occur in a normal variation of posterior circulation blood supply. The infarcted areas include the rostral (front) portion of the midbrain and the ventromedial aspects of both thalami (ref 1). Although these areas are affected in this case, there are also infarcts in the left cerebellar hemisphere and the right occipital lobe more in keeping with a wider posterior circulation stroke.
Extensive posterior circulation infarct with signal abnormality and restricted diffusion in: both medial thalami, midbrain, both dentate gyri, both occipital lobes, left cerebellar hemisphere. Small calibre posterior circulation vessels, but no absent signal voids.
