The patient stated the lesion was painful. Is there any correlation between pain or lack thereof and the presence or absence of malignancy?

None. Some cancers are painful and some are painless. No predictive value.
You see this if you do lots of breast biopsies. For some reason a very small percentage of cancers are exquisitely painful on biopsy: some malignant lesions you can freeze all you want the patients experience excruciating pain. Other malignant lesions the patient is almost unaware of what is happening. Puzzling.
