High Intestinal Obstruction:

  1. Upper Obstruction (< 3 dilated loops), vomiting is non-bilious if proximal to the ampulla of vater.
    • Duodenal atresia
    • Duodenal stenosis
    • Duodenal web
    • Annular pancreas
    • Malrotation & midgut volvulus
  2. Low Obstruction
    • Hirschsprung
    • Meconium plug syndrome
    • Ileal atresia -> microcolon on contrast enema
    • Anal atresia/anorectal malformation
    • meconium ileus (CF only) - microcolon on contrast enema

Bilious vomiting -> obstruction must be distal to

Note: meconium ileus only in CF.

Premature infants -> NEC

Hypertrophic pyloric stenosis = projectile vomiting in otherwise normal infant (1 week to 3 months)
