What is the differential diagnosis for a 'flitting' pneumonia?
Eosinophilic pneumonia, aspiration pneumonia, ABPA, and COP.
This patient was not asthmatic, has normal swallowing and oesophageal function, and has a normal white cell differential. What is the likely diagnosis?
Cryptogenic organising pneumonia
Over the months, the apperances are that of migrating/("flitting") infiltrates. Prior sympathectomy for Raynaud disease noted.
A prior CT chest (not available for review) did not show any mass lesion, and no structurally abnormal lung.
