What condition does this MRI show?
This is a case of multiple sclerosis, with areas of active demyelination.
What are "Dawson's Fingers"?
This is a term used to describe the white matter changes that spread up from the ventricles - usually in a perpendicular, radial distribution. It is a phenomenon seen in multiple sclerosis, but is not a diagnostic feature.
What criteria is used to diagnose multiple sclerosis?
The McDonald Criteria is an internationally recognised guideline to help diagnose MS. There are two main aspects: dissemination in time and dissemination in space. Having a history of neurological deficits, new lesions or enhancement with contrast helps to show dissemination in time. Having a number of lesions in different areas of the brain and cord is needed for dissemiation in space.
Multiple lesions throughout the brain with high T2 signal. Different grades of enhancement, and two active lesions with avid contrast in the right periventricular white matter consistent with the left-sided sensory changes.
There are lesions in the periventricular, juxtacortical, brainstem and cerebellar spaces as well as the cervical cord.
SUMMARY
Active multiple sclerosis.
