Acute primary intraventricular hemorrhage with triventricular hydrocephalus.  This is likely to be hypertensive in origin in this patient.

The intraventricular hemorrhage illustrates well the ventricular system of the brain, in particular the cerebral aqueduct and the lateral apertures of Luschka.
