There is round slightly high on T1WI (higher than CSF) & bright on T2WI mass at the roof of 3rd ventricle (foremen of Monro) measuring 14 mm, there is mildly peripheral enhancement following IV contrast injection; it causes obstruction of foremen of Monro & moderately dilatation of lateral ventricles & transependymal CSF flow (obstructive hydrocephalus). The picture is typical for a colloid cyst causing acute obstructive hydrocephalus; emergency shunting is needed.
Case Discussion
The patient went on to have a resection which confirmed the diagnosis of a colloid cyst.
