Coxa brevis

Case contributed by David Horvath
Diagnosis almost certain

Presentation

Unilateral hip-impingement like symptoms with maximal pain and fatigability on hip abduction since birth.

Patient Data

Age: 30 years
Gender: Female
x-ray

Longitudinal shortening of the femoral neck, with a shortened hip lever arm ratio.

Case Discussion

A shortened hip lever arm impedes hip abductor muscle efficiency. Consequently, the hip abductors must generate more force to stabilise the pelvis, which contributes to fatigue and pain.
