Lateral ankle ligament injury

Diagnosis almost certain

Presentation

Left ankle inversion injury one week prior. Lateral ankle pain and swelling since. A fracture was radiographically excluded.

Patient Data

Age: 25 years
Gender: Male
ultrasound

Anterior talofibular ligament

  • ​sagging of the ligament with discontinuity in its mid part

Calcaneofibular ligament

  • discontinuity in mid part the ligament with pseudo mass formation
  • perineal tendons are in a deeper plane in comparison to the ligament; which normally lie superficial to the ligament.

Anterior inferior tibiofibular ligament

  • intact with normal echo pattern

Anterior recess of the ankle joint

  • no effusion

Asymptomatic ankle

  • intact with normal echo pattern of anterior talofibular and the calcaneofibular ligaments

Case Discussion

Lateral ankle collateral ligament injury is common in athletes as well as in non-athletes.

Isolated anterior talofibular ligament injury is more common than the combined injury of anterior talofibular and calcaneofibular ligaments. Isolated injury of the calcaneofibular ligament is rare. 

The case shows complete tear of the anterior talofibular ligament and the calcaneofibular ligament in a young adult.
