MCA infarction with subarachnoid FLAIR vascular hyperintensities

Presentation

Dizziness and recent onset of right side body weakness

Patient Data

Age: 40 years.
Gender: Male

Left cerebral areas of recent infarction are seen involving the frontal, parietal and temporal lobes as well as left caudate and lentiform nuclei, in the form of abnormal high signal on DWI and corresponding low signal on ADC. On FLAIR, there is loss of normal flow void at the M1 segment of left middle cerebral artery (MCA) with corresponding signal drop on gradient sequence denoting MCA thrombus. Distal to the thrombus, multiple subarachnoid vascular hyperintensities are seen on FLAIR.

Bilateral cerebral old ischaemic changes are also seen.

Case Discussion

FLAIR sequence can demonstrate the sluggish flow along the small vessels in the subarachnoid space as hyperintensities, which is reported in cases of acute ischaemic stroke.
