Presentation
Ca (carcinoma) bladder patient with obstructive symptoms and haematuria.
Patient Data
A large (72 cc = 5.8 x 4.4 x 5.4 cm in size) hypervascular, predominantly solid tumour with homogenous to mildly echogenic reflectivity and irregular outer borders, is noted within the intravesical bladder lumen abutting more towards the right ureteral orifice. The lesion is inseparable form the inner bladder mucosa but is well delineated from the prostate gland which measures 30.02 grams = TRUS measurement. Subtle bilateral pelvicalyeactasis is noted.
Histology:
Urothelial carcinoma. Sarcomatoid variant. High grade. No lymphovascular infiltration. Tumour extension to muscularis propria.
Case Discussion
Clinically confirmed muscle invasive bladder carcinoma post urethrocystoscopy plus direct visual urethrotomy (DVU). The prostate gland is not involved. Low level intravesical bladder debris noted could be attributed to the current on going haematuria.
