Diagnosis almost certain

Presentation

Recent dysuria. Incidental finding during ultrasound.

Patient Data

Age: 40 years
Gender: Female
ultrasound

A solid lesion (65 x 48 x 43 mm) replaced the right ovary. The lesion shows a few tiny echogenic foci (<1 mm) without acoustic shadow, surface irregularity, and moderate vascularity

Uterine myometrial heterogeneous echopattern with thicker posterior wall. Normal endometrium. Normal left ovary with a dominant follicle.

No pelvic nodes / peritoneal thickening/ ascites. The rest of the abdominal ultrasound was normal.

Case Discussion

Incidentally detected ovarian mass shows features of malignancy. Histopathology revealed right ovarian mass as poorly differentiated carcinoma along with uterine adenomyosis. Immunohistochemistry suggested high-grade serous ovarian carcinoma.
