Presentation
History of birth asphyxiation presented with delayed motor development.
Patient Data
Asymmetrical dilatation of the left lateral ventricle compared to the right with an irregular wavy margin of the body of the left lateral ventricle.
High T2/FLAIR signal intensity in the periventricular subcortical white matter bilaterally extending to the posterior limb of the internal capsule inferiorly and centrum semiovale superiorly on the left with few cystic changes adjacent to the body of the right lateral ventricle and a small focus of high signal intensity in the junction between the right thalamus and posterior limb of the internal capsule.
Focal thinning in the posterior part of the corpus callosum body.
Overall features are suggestive of periventricular leukomalacia.
Case Discussion
Periventricular leukomalacia is a pattern of hypoxic/ischaemic brain injury affecting mainly the periventricular white matter and occurring in preterm infants, with patients presenting cerebral palsy-like symptoms, interstitial disability, or visual disturbances.
This patient presents with an inability to walk and a spastic right arm.
