Peroneus brevis tear

Diagnosis almost certain

Presentation

Left ankle injury - two months ago. Complaining of lateral ankle region pain and occasional snapping.

Patient Data

Age: 35 years
Gender: Male
ultrasound

Longitudinal separation of the peroneus brevis tendon in the retromalleolar groove region and extending just distal to the lateral malleolus. The length of the split tear is about 35 mm. No tenosynovitis. Normal proximal muscle belly. Distal tendon insertion is normal.

Peroneus longus tendon and muscle belly are normal. No injury of the peroneal retinaculum. No brevis/ longus tendon subluxation/ dislocation in relation to the retromalleolar groove.

Calcaneofibular ligament is normal.

Case Discussion

The case shows a peroneus brevis tendon split tear.

The presence of three structures in the retromalleolar groove raises two possibilities. One is the split tear of the tendon and the other is an accessory peroneal muscle.
