Pure matrix renal stones

Case contributed by Mohammad walid Ahmad Amin
Diagnosis almost certain

Presentation

Recurrent bilateral flank pain.

Patient Data

Age: 40 years
Gender: Female

No obvious dense renal stones.

ultrasound

Multiple bilateral renal hyperechoic foci with posterior acoustic shadowing and twinkling artifact, suggesting renal stones.

Case Discussion

Almost all renal stones are radiodense and can be visualised through radiography or computed tomography. However, there are two rare types of renal stones that appear radiolucent: pure/protein matrix stones and some medication-induced stones.

Matrix stones may appear echogenic on ultrasound however acoustic shadowing and twinkle artefact is variable and probably depends on the extent of mineralisation.
