Pyelonephritis

Presentation

High-grade fever associated with abdominal pain and vomiting.

Patient Data

Age: 12 years
Gender: Female

There is heterogeneous, abnormal cortical hypoenhancement of the right kidney with mild surrounding perinephric stranding/inflammation in comparison to the normal-appearing left kidney. In the upper pole of the right kidney, there are a few slightly more focal appearing, rounded hypodensities. There is subtle urothelial enhancement of the right ureter, however without frank ureterectasis.

Case Discussion

This is a case of pyelonephritis in a paediatric patient. Given the round densities, there was question of developing microabscesses.

The patient was treated with IV antibiotics and then eventually transitioned to oral antibiotics. Her treatment course was uncomplicated.
