Right upper lobe pneumonia

Presentation

Productive cough and fevers for one week.

Patient Data

Age: 61
Gender: Male
x-ray

There is airspace consolidation in the peripheral right upper lobe with associated air-bronchograms.

There is an area of air space consolidation in the posterior segment of the right upper lobe measuring 45 x 47 x 44 mm. The lesion is pleural based laterally and shows some air-bronchograms. There is a central poorly defined area of hypo density suggestive of necrosis. A poorly defined halo of ground glass density around the consolidation is seen.

The chest radiograph shows further resolution of the right upper lobe consolidation after 4 weeks with a minor area of sub segmental atelectasis noted.

Case Discussion

There is an area of right upper lobe air space consolidation. This is thought be caused by infection / pneumonia. A repeat x-ray in 4 weeks is recommended to confirm resolution of consolidation. If the consolidation persists after antibiotic therapy, then other differentials such as inflammatory non-infectious and neoplastic aetiologies are included in the differentials. 

Repeat chest x-ray in 4 weeks showed resolution of the right upper lobe consolidation with oral antibiotics.
