Sciatic nerve entrapment by soft tissue tumour

Presentation

Right foot drop for the last few days. No recent trauma.

Patient Data

Age: 20 yrs
Gender: Male

A well-defined ovoid lesion (58 x 41 x 33 mm) in the posterior mid-third of the right thigh; at the site of the clinically palpable lump under the surgical scar. Heterogeneously hypoechoic lesion without cystic changes/ calcification/ flow signals. The lesion elevates the sciatic nerve as seen in the long-axis view. The short-axis view shows the sciatic nerve lying within the peripheral part of the lesion. There is rim-like hypoechoic soft tissue thickening around the sciatic nerve extending distal to the lesion for about 20 - 30 mm.

Case Discussion

A known case of right thigh round cell tumour. Post-chemotherapy, post-radiotherapy, and post-surgery status. Presented with a palpable lesion in the posterior thigh near the scar region and foot drop for the last few days. Referred for an ultrasound to check sciatic nerve entrapment by tumour recurrence.

With the available clinical background, ultrasound findings favour the possibility of local recurrence of soft tissue tumour in the thigh with entrapment of the sciatic nerve causing foot drop.
