Presentation
Pain after rugby tackle, unable to weight bear on knee.
Patient Data
There is an avulsion fracture of the lateral tibial rim, also a comminuted intra-articular fracture of the tibial plateau that involves the tibial spine. A lateral femoral notch sign is also present.
Fullness of the suprapatellar bursa shadow on lateral view with fat pad separation, suggestive of knee joint effusion.
Case Discussion
Segond fractures have a high association with ACL injuries, fractures like these should be considered for further imaging to assess the integrity of the ACL.
This particular case demonstrates a fracture of the tibial spine, highly concerning for a significant ACL injury.
Unfortunately, this patient went on to have further imaging at a different hospital hence, the follow-up MRI is not available.
