Motorbike accident, handle bar to epigastrium. History of total colectomy with end ileostomy for ulcerative colitis

Patient Data

Age: 60 years
Gender: Male

Traumatic rupture of the left diaphragm with herniation of the gastric fundus into the thoracic cavity.

There is also an AAST grade 1 splenic laceration anteriorly.

Case Discussion

Diaphragmatic rupture should be managed operatively due to the risk of strangulation. There are also usually other injuries. In this case, he also had a grade 1 splenic laceration.

This patient proceeded to have a laparotomy to reduce the hernia and repair the rupture. He also had a splenectomy due to ongoing bleeding from the splenic laceration.
