What are the findings on this chest X-ray?
There is a large mass in the right mid zone. Loss of the right heard border indicates this mass is abutting the right heart and is therefore either in the anterior mediastinum or the right middle lobe, however because the right hilar vessels can be seen through the mass, it is an anterior mediastinal mass and not a hilar mass. This is known as the hilum overlay sign. Atelectatic change within the right lower lobe (the right hilum is pulled inferiorly with right sided volume loss and there is a triangular shaped density in the right lower zone). Bilateral pleural effusions indicated by the blunting of both costophrenic angles.
There is a large mass in the right anterior mediastinum. Atelectasis in the right lower lobe. There are bilateral pleural effusions. The left lung is otherwise clear. No bony abnormality.
